The documents you'll need for a credit life claim depend on the claim type. All claims need a signed claim form, a certified ID copy, and your signed loan agreement and policy schedule. Death claims add a death certificate; retrenchment claims add a retrenchment letter and UI-19; disability claims add a medical report.

Documents you'll need for a claim

The supporting documents depend on the claim type. Here is the full checklist for each:

    Death claims

    • Completed and signed claim form (we'll provide it).
    • Certified copy of the deceased's South African ID.
    • Certified copy of the official death certificate from Home Affairs.
    • Signed loan agreement.
    • Signed credit life policy schedule.
    • Settlement statement from us (we'll provide it on request).
    • Police report — required only if the cause of death was unnatural (accident, homicide, etc.).

    Retrenchment claims

    • Completed and signed claim form.
    • Certified copy of the claimant's ID.
    • Retrenchment letter on the employer's letterhead — OR a certificate of service if more than two months have passed since you took the loan.
    • Completed UI-19 form (issued by the employer at termination).
    • Most recent payslip.
    • Signed loan agreement and credit life schedule.
    • Settlement statement from us.

    Permanent disability

    • Completed and signed claim form.
    • Certified copy of the claimant's ID.
    • Medical report from a registered practitioner confirming the diagnosis and that the disability is permanent.
    • Letter from your employer confirming loss of income.
    • Signed loan agreement and credit life schedule.
    • Loan statement and settlement statement.

    Temporary disability

Same as permanent disability, plus evidence (usually a letter from your employer) that you are financially impaired during the disability period.

Don't worry if you don't have everything ready on day one. Submit what you have; our team will tell you what's still outstanding.
